CSB Wellness offers mental health and other wellness services to adults. Individuals can initiate services regardless of background, legal status, or orientation. CSB Wellness accepts self- and provider-referrals for services.
All services begin with a free initial consultation. Depending on the service, individuals have flexible options for in-person and telehealth services. -
Claire Schulz Bergman maintains a professional license in the State of Wisconsin. She also maintains certifications corresponding with any professional work at CSB Wellness and within other agencies.
Claire is the sole provider affiliated with CSB Wellness and CSB Wellness, LLC. However, Claire works with other agencies, organizations, and other providers, who act as their own independent entities.

Claire Schulz Bergman is licensed to provide psychotherapy sessions only to clients who reside in Wisconsin. Health coaching and yoga therapy have no residential limitations and can be provided to out of state individuals through the telehealth platform.
CSB Wellness only offers services to adults (ages 18+).
CSB Wellness is not a good fit for individuals who are acutely symptomatic, require ongoing crisis intervention, or are assessed to be in need of a higher level of care. In these situations, CSB Wellness makes efforts to assist individuals as they get connected for services.

Psychotherapy primarily addresses mental health concerns through in-depth exploration of psychological issues, while health coaching is focused on promoting overall well-being, often through lifestyle changes and preventive measures. Both approaches can be valuable depending on individual needs, and in some cases, they may complement each other when addressing the interconnectedness of mental and physical health.

The length of services is highly individualized and based on the wellness modality. Individuals who conclude services have the option to resume if or when needed and as appropriate.
Service Fees & Pricing

I currently have openings for both in-person and telehealth services. Fees are listed as follows:
Individual Therapy Session (50 min): $175
Individual Therapy Session (30 min): $110
In an effort to provide accessible counseling and affordable rates, I also reserve a portion of my appointments for those who need sliding scale rates and would be happy to discuss ways to make therapy more affordable to you.
All fees are commensurate with my credentials, training, specializations, and community standards.

Every coaching package is completely customized to meet your needs. Each session is 50 minutes of pinpointed coaching, strategy, and education that is tailored to your goals, what your body needs and what you want to achieve. Sessions include exploration of gaps, identification of goals and co-creating a plan for success. Coaching is most successful when sessions are spaced relatively close together.
Coaching Packages:
Single full session (50 min) = $175
Single half session (30 min) = $110
6 full sessions = $900 ($150 off!)
12 full sessions = $1850 ($250 off!)
Packages include reasonable access to me via text or email in between sessions. Packages expire after 6-months.
Other Financial Information

CSB Wellness is not affiliated with any particular insurance panel at this time. Therefore, any service at CSB Wellness is considered “out of network.” This is to ensure the greatest level of privacy, flexibility and control of your treatment and mental health records.
Upon request, individuals are provided with a superbill, which documents the services provided and payment rendered. Individuals are responsible for submitting documentation for reimbursement to their respective insurance providers. At this time, CSB Wellness does not submit documents to insurance. Additionally, existing clients are able to access service invoices via a secure portal to their electronic medical record.
Confirming Out of Network BenefitsIndividuals are responsible to confirm out of network benefits and options for reimbursement with their insurance provider. This is done by calling the number on the back of your insurance card and asking for the reimbursement rate. Please inquire for billing codes.
Subject to Change
If at any time throughout your work here, CSB Wellness becomes paneled with insurance or uses a third-party to assist with reimbursement, you will be notified. However, CSB Wellness reserves the right to continue any existent payment and financial options and those put forth at the time services were initiated.

Payment is due at the time of service, unless otherwise agreed upon. Payment information and schedule of fees is outlined at the outset of service or if individuals have a change to their financial status over the course of therapy.
All major credit cards and HSA/FSA cards are accepted. Individuals add their financial information to their secure portal prior to the initial session. CSB Wellness does not ever possess, retain or upload credit card information.

CSB Wellness offers packages for purchase and membership-style financial options. Membership options include a one-time, monthly payment for services. Membership options are billed at the beginning of the month. Please inquire for additional information.

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
